If such an option existed, my reply would be:
Only once, really. With the encouragement of my boss, because the organization's parent company was being taken over by Bastards, and we were losing our Jobs. (These would be the same Bastards who later somehow fixed it so I couldn't claim the time I'd worked there for unemployment, in spite of the fact that we knew from the accountant that unemployment had always been paid into for me/us.)
Taking of occational supplies from my current company doesn't count as stealing, because the Boss tends to give them to us if we want/need them, even if we offer to pay for them (as in the case of the comb bindings/use of the comb binding machine).

I have a tendancy to walk off with pens, although that's more a matter of accident.
The last place I worked did formating for text books, the walls were lined with finished books, which they gave away once a year to the people who wanted them. Sadly, I was gone before I got a chance.
So many free text books...

at my previous job, I ordered pen refills for my retractable pens, which no one else in the office used. I took those along with some paperclips that were already stuck to my Paper Clip Holder when I left the agency for my current job.
In general, I tend to buy my own supplies and bring them in to work for me to use, but I have made copies for personal reasons on several occasions.

I stole a stapler and some white-out from my 11th grade english teacher cause he was a bastard.
umm, to a teacher, a school is an office, so they were office supplies, they were just stolen in a school.
That's irrelevant. The question is not "have you ever stolen supplies from your office?" The supplies were office supplies, they were stolen. The end.

I've used some scrap material and the tools at work to build a couple things for myself. If that doesn't count as office supplies, then all I've done is forgotten to take pencils out of my pocket and brought them home by mistake.
